Complementary Surveys

There are lots of situations where a simple survey would give you a good start toward a more serious and successful change effort. The surveys provided here meet a variety of common situations. You are welcome to use the questions or modify them to meet your specific needs. The surveys are written in plain text so you can easily insert them into your own word processor.

Each file also contains suggestions for formatting the survey as well as analyzing the data. Since the number of questions is typically small, it is possible to get substantial value from even informal data analysis techniques. A basic fluency in Excel is presumed; you will also want to use the Data Analysis package available as part of the program's add-in's.

Cross Functional Teams: (.txt format) The behaviors, disciplines and attitudes that support productive and enjoyable group activities; clarity of roles; conflict management; decision making; etc.

This survey is appropriate for teams consisting of members of different organizational groups. Typical examples would be cross functional task forces, product development teams, or process improvement teams.

Team Leader: (.txt format) This survey provides feedback to a team leader. It is appropriate as a support for development efforts. It would NOT be appropriate to use as part of their formal evaluation or bonuses.  Such practices inevitably distort the data and lead to defensiveness rather than acceptance of the data.

Management Feedback: (.txt format) For upward feedback in support of individual management development efforts. It would be appropriate as a support for development efforts. It would NOT be appropriate to use as part of their formal evaluation or bonuses. Such practices inevitably distort the data and lead to defensiveness rather than acceptance of the data.

Meetings: (.txt format) This survey provides feedback to a group on the quality of their meetings. It might be used with a single group (such as a staff group or departmental committee) or for a larger group interested in improving the quality of their meetings in general.
